COLOR LOCK SYSTEM
Safety and Lockout Standards (LOTO)
🔴 Red - Personal Lock
WHO USES: Every worker
WHERE: on Lock Box ONLY with a personal tag (this is critical!)
HOW MANY: One per person (your personal life-saver)
🟡 Yellow - Isolation Lock
WHO: AIO personnel only (requires specific certification)
WHAT: Mechanical lock on energy source
WHERE: At point of isolation (not on Lock Box)
⚫ BLACK - Master Lock
WHO USES: Isolating AIO only
WHERE: Lock Box exterior (never on equipment)
KEY SYSTEM: Individual unique key per AIO (registered and tracked)
🟢🔵 SPECIAL
WHY: Creates first layer of protection
KEY RULE: Your key = your responsibility
WHEN: After AIO isolation complete
WHY: Individual safety assurance
HOW: Applied after de-energization, before testing
🟢 Green - Commissioning Lock
WHO USES: Pre-Commissioning team (special authorization)
WHERE: Equipment being tested (energized!)
🔵 Blue - Construction
WHO USES: Construction supervisors/managers only
WHERE: Boundary isolation points (construction zone perimeter)
KEY SYSTEM: Uses a common-key system for authorized personnel
WARNING: Equipment is LIVE - not isolated!
SAFETY: Barricades + signs + cleared areas required
NEVER: Use for maintenance or as isolation substitute
PURPOSE: Secures all isolation keys inside Lock Box
TIMING: Applied after verification, before workers lock on
CRITICAL: Removal = permission to re-energize equipment
NEVER: Leave keys accessible or share with others
KEY SYSTEM: Individual unique key per authorized supervisor
PURPOSE: Separates construction zones from operational areas
CONTROL: Managed by Construction Manager/Director
NEVER: Use for equipment isolation or personal protection
NEVER: Use for equipment isolation or personal protection
LOTO: Isolation Procedure
Before a worker can place their red lock, the AIO must perform several critical actions.
1. Identifying Energy Sources
AIO finds ALL sources: electricity, control panel, backup generator.
2. Physical Isolation
AIO physically disconnects everything and places YELLOW locks.
3. VERIFICATION
A critically important step before locking out.
AIO attempts to start the equipment (e.g., fan).
Result: The equipment does NOT work.
4. FINAL LOCKOUT
Keys from yellow locks → into Lock Box
AIO locks the Lock Box with a BLACK lock
Result: Access to isolation keys is physically blocked.
5. YOUR PERSONAL STEP
You place your RED lock
Where: On this same Lock Box
Safety: No one can access the breaker keys until you remove your red lock.
Work calmly — no one can turn on the fan while ALL locks are in place.
