Lockout / Tagout Standards for Industrial Safety

What is LOTO?

A specific safety procedure to ensure dangerous machines are properly shut off.

Prevents accidental release of hazardous energy during maintenance.

Controls electrical, mechanical, hydraulic, pneumatic, and thermal energy.

Fatalities by Energy Type Absence

Failure to control hazardous energy accounts for nearly 10% of serious accidents. This chart shows incidents attributed to missing LOTO procedures.

Authorized Isolation Officer vs. Worker

<b>Authorized Isolation Officer (AIO):</b><br>Trained, competent person who isolates the energy sources and verifies zero energy. They place the 'System Locks'.<br><br><b>The Worker:</b><br>Verifies the AIO's work and places their personal RED lock on the Group Lock Box. They do clear tasks but do NOT manipulate valves or breakers.

CRITICAL RULE: Workers do NOT isolate energy. Workers only lock the Lock Box after verification.

Safety Golden Rule

1. Preparation of documentation

<span style='color:#D62828; font-weight:bold;'>RED LOCK:</span> Personal Lock. Applied by every worker involved in the task to the Lock Box.

<span style='color:#DAA520; font-weight:bold;'>YELLOW LOCK:</span> Transition / Shift Handover Lock. Used when transferring responsibility or during shift changes.

<span style='color:#32CD32; font-weight:bold;'>GREEN LOCK:</span> Master Lock. Used by the AIO to secure the Lock Box keys or specific control points.

<span style='color:#000000; font-weight:bold;'>BLACK LOCK:</span> Commissioning / Special Operations. Used for testing or non-standard isolation scenarios.

<span style='color:#1E90FF; font-weight:bold;'>BLUE LOCK:</span> Equipment Lock. Applied by AIO to the isolation points (valves, breakers).

Prepare the document package: <b>Permit to Work (PTW)</b>, <b>Isolation Certificate</b>, and <b>Job Hazard Analysis (JHA)</b>.

Initiator

Manager, work responsible person, supervisor, or person in charge. Submits the work request, specifying scope, location, and required isolations.

Permit Issuer / Authorizing Manager

The person empowered to issue permits (PTW), preparation of JHA and Isolation Certificate. Verifies site conditions, coordinates with other work activities, and validates the proposed isolation plan. Signs and issues the PTW.

2. Authorization of personnel

Permit Acceptance

Accepts the PTW, ensures understanding of all its conditions, and is responsible for their enforcement at the worksite.

Work Responsible Person (Supervisor)

Provide toolbox talk with the work team, inform personnel about the job-specific hazards, related risks, and required control measures. Ensure the JHA is reviewed and signed by all members of the work team.

3. De-energization & Isolation

Perform full physical isolation of all hazardous energy sources.

Authorized Isolator Officer (Primary Responsible)

Directly performs the LOTO procedure - shuts down the equipment, isolates all energy sources, and applies their personal locks and tags.

Work responsible person, supervisor, person in charge

Ensures all hazardous energy sources have been identified, and isolation plan is correct. Authorizes LOTO.

Verifying Role: Safety Officer / LOTO Auditor

Conducts an independent verification (audit) of the completeness and correctness of the isolation, especially for complex systems.

Isolation Procedure: Step 1-3

1. <b>Preparation:</b> AIO identifies all energy sources (Electrical, Mechanical, Pressure) using the LOTO Permit.<br><br>2. <b>Notification:</b> Inform all affected employees in the area about the impending shutdown.<br><br>3. <b>Shutdown:</b> Perform the normal machine stop sequence to avoid kickbacks or damage.

4. Implementation & Verification of Control Measures

<p style="margin-bottom: 20px;">Implement and verify all critical control measures before work begins.</p> <p style="margin-bottom: 20px; color: #4facfe;"><strong>Primary Responsible:</strong> Work responsible person (supervisor).</p> <ul> <li style="margin-bottom: 15px;">Confirm the implementation and effectiveness of all defined critical control measures specified in the PTW and JHA are implemented at the site before work commences.</li> <li>Verify the effectiveness of lockout/tagout devices and other energy control measures to prevent accidental energization or hazardous energy release.</li> </ul>

LOTO Procedure Flowchart

Every worker must place their PERSONAL RED LOCK on the box before starting work. This ensures the keys cannot be retrieved until every single worker is safe.
